Dear Provider:
Attached is the CREDENTIAL AMERICA© General
Application. This document is used so
we may gather much needed information for credentialing purposes. Please make sure you answer every question
possible and provide us with any additional information you feel we need on a
separate piece of paper.
In addition to sending us your completed CREDENTIAL
AMERICA© General Application, we need to obtain a listing of the health
plans for which you need to be credentialed.
We need both items before we can proceed with making phone calls to the
health plans for you.
Please do not hesitate to call us with any questions. Below is a list of staff members that you
will be working with along with phone numbers and e-mail addresses for each:

From the moment we receive your information, signed
contract and payment, we will begin contacting the health plans on your behalf
to request applications. Then, as they
are received, we will complete them and send them to you for signature. We ask that you review the applications for
accuracy and sign and date where indicated.
Then, make a copy for your records if you’d like and forward them to the
health plans in the envelopes provided.
Of course, it is always best to send them certified mail or overnight
delivery to insure receipt. The cover
letter that accompanies your completed application(s) will instruct you to
document the date you sent the application(s) and the method of delivery. You will need to fax this letter back to us before
follow-up will commence. This
eliminates unnecessary follow-up phone calls for us in case you have not yet
sent your application to the credentialing entity.
The staff of Credential America© understands
the importance of credentialing and we will do everything possible to expedite
the process. However, you should be
aware that the credentialing process can take several months. We have no control over the length of time a
health plan takes to credential you.
But, be assured that we will be following up with them to be certain
they have all the information necessary.
Upon request, you will receive a weekly status report that
will provide you with a summarization of your status with each plan. This report is in no way representative of
all of the work we have performed on your account. We keep comprehensive notes regarding your progress and the
conversations we have with health plan representatives. However, this information is much too
lengthy to include in a report. If you
ever decide that you need more detailed information about the status of a
health plan, simply call us and we will be glad to provide that information to
you.
